Education, Health and Care Plans

An Education, Health and Care Plan (EHCP) is a legal document that describes a child or young person’s special educational needs, the support they need, and the outcomes they would like to achieve.

An EHCP includes:

  • A detailed assessment of a child/young persons need
  • The support, additional or specialist provision a child/young person requires to meet their needs
  • The outcomes that this provision should help them to achieve

Process

  • The Local Authority receives a request for EHC Needs Assessment. A Request for Assessment can be made by either a parent/carer, an educational setting or a professional involved with the child or young person. 
  • An EHC Needs Assessment is carried out and a decision is made about whether an EHCP is required
  • An EHCP is agreed and steps taken to meet the provision laid out in the plan.
