Our Learning Disability and Autism Social Work Team supports adults in Sefton who have a diagnosis of a learning disability, autism or both.
What does our team do?
The team includes:
- A Team Manager - This person is in charge of the team and makes decisions about the team.
- A Lead Practitioner - This person will guide the team and help them in their job.
- Social Workers - They will work with you to decide what help you need and put plans in place.
- Community Care Assessors - They check what support you need to live in your own home.
They also work closely with other health and social services to give the best help to people.
A social worker will work closely with you and your family to find out your:
- Care needs.
- Daily living and social needs.
- Life skills.
- Goals for the future.
- Employment and training needs.
- Things you're struggling with.
After this, they will make a support plan to help you. The plan will be based on the Care Act and will give ways to help you with:
- Eating healthy.
- Keeping clean and using the toilet.
- Wearing the right clothes.
- Keeping your home safe.
- Having good relationships with family and friends.
- Going to work, school, or volunteering.
- Using local services like buses, parks and leisure centres.
- Caring for children or family.
- Staying safe from abuse or neglect.
The team will then help you to live a full and healthy life. They will help you to reach your goals. They will always listen to what you want.
How to access the service
You can access our service in different ways. You can ask for help yourself, or a family member can ask for help for you. A professional can also make a referral if they think our service can help you.
If you have been helped by our Children’s Social Care team, they will pass you to our team before you become an adult.
Once we receive your referral, we’ll see if we can help or if someone else can. If we can help, we’ll get in touch with you and set up a time to meet with us.
