  • DWP Supporting Families team are 2 secondees who work with adults in the family who are on an EH, CP, CIN or YOT plan.  
  • The team support the family to gain employment or make progress to work (e.g., gained a qualification, completed training, volunteering, is attending job interviews) 

Early Help Assessment: Coordinated support with a TAF whole family. 

  • Housing Options Advisors will support those who are within 56 days of being made homeless and those who are currently homeless. Families with Children who have a local connection will be treated as priority need and will be offered temporary accommodation in the interim while being supported to move on into stable accommodation. Families can self-refer via Sefton Council's website or be referred in by professionals via Duty to Refer portal located on the website. 
  • Joint working between the Leaving Care Team and Housing Options to ensure that care leavers are supported into suitable accommodation.  

Early Help Housing Practitioner: Working directly with families on EHP, TAF, CIN, CP and YOT plans to resolve housing issues e.g., working with Housing Standards, HOT, SWACA, Links with PPP for application and activation, Eviction Notices, rent arrears, Landlord negotiations, access to Legal Advice, Homelessness Prevention, tenancy sustainment, move-on options, benefits assistance, household budgeting, onward referrals to wider agency support. 

  • Sefton at Work supporting Families team are 2 secondees who work with adults in the family who are on an EH, CP, CIN or YOT plan.  
  • The team support adults to gain employment or make progress to work (e.g., gained a qualification, completed training, volunteering, is attending job interviews).  

For clinical input for young people who can't or won't attend the service, there is a team of clinical outreach nurses who can provide contraception and STI testing and treatments.   • Intensive Support for Families in need: Our aim is to deliver support 2/3 times per week between 12-20 weeks to families in need of support. Focussed areas of work with completion of such items as Frequency, intensity, duration, and onset (FIDOS) Safety plans, aims and goals, strengths and needs. Intensive 2 - 3 visits per week to home, school, places of attendance.  
  • 12 - 20 weeks support offer: We work with the whole family ensuring that all family members are supported and included. Case mapping with all stakeholders and family members.  
  • Case referrals accepted from Childrens social care via C&F, step down from CIN. We support families to make positive changes for a better future. Use of sustainability model to ensure supportive transitions from support. Ensuring stakeholder involvement, Family are at the heart of their plan, engaging all parties.
