Please read this general guidance before completing your nomination(s).
There are 12 different award categories open for nominations this year and you can make all nominations via the e-form.
- All nominees must live in, or have contributed to sport within Sefton
- All information must be relevant to the previous 12 months
- You can nominate an individual or club in more than one category
- The judging panel can only make their decision based on what is written in the nomination form
- The closing date for nominations is May 10. There will be no extension to this
